What is BPH?
The cause of this type of cancer is not known, but is probably due to hormonal disturbances that occur with aging. The pathogenesis plays only an initial role, leading ultimately to a violation of hemodynamics of the bladder and kidneys, and naturally developing tissue hypoxia. Hypoxia of detrusor in BPH is not only a consequence of the relative lack of blood flow in hypertrophied bladder wall and compression with adenomatous nodes of lower cystic artery, but also a consequence of the age atherosclerosis arterial disease (“detrusor senile”). BPH leads to the development of bladder outlet obstruction which eventually leads to high detrusor pressure and reducing the flow of urine from the kidneys and the emergence of vesicoureteral reflux and kidney.
These phenomena trigger ischemic processes in the kidney tissue, which are the important pathogenetic link in the development of inflammation and renal failure. Many men over the age of 50 years have some kind of disorder of urination, i.e. symptoms are typical of BPH. However, the cause of the symptoms which are specific to adenomas may be not only the disease but prostate inflammation (prostatitis), prostatic cancer, impaired contractility of the bladder muscles. Therefore, men older than 40 years should promptly contact the urologist so that doctor could make the correct diagnosis and prescribe the necessary treatment.
What Are the Signs and Symptoms of BPH?
BPH occurs due to proliferation of the body tissue, wherein an increase in prostate volume. This leads to compression of the prostatic urethra and a constriction of its lumen, which prevents normal outflow of urine from the bladder and promotes obstructive symptoms:
- difficult urination;
- sluggish stream of urine;
- increased urination time;
- retention, combined with its incontinence (paradoxical ischuria).
Simultaneously symptoms of diseases caused by changes in the bladder muscle appear, and irritation of the nerve endings of prostatic urethra increases, this is symptoms:
- frequent nighttime awakenings for urination;
- frequent and painful urination in small doses;
- withhold urine with urge to urinate.
Major complications arising for patients with BPH as follows:
- residual urine in the bladder;
- acute urinary retention;
- bladder stones;
- chronic cystitis (inflammation of the bladder);
- kidney inflammation;
- chronic renal failure;
- hematuria (blood in the urine).
The stream of urine becomes thinned and very sluggish, the patient may feel that the bladder is not completely emptied; residual urine volume reaches 100-200 ml or more. At this time, acute urinary retention can occur, and because of the difficulty of outflow of urine from the upper urinary tract is disturbed renal function.
